Data Deletion Request

This page explains how to request deletion of your account and data from the RIAJ mobile app and website, operated by Margaret Bowen MD PLLC (Restoration is a Journey).

How to Request Deletion

In the app. Open the RIAJ app, go to Settings, and select Delete Account. Confirm the deletion when prompted. This request is permanent and cannot be undone.

Please include your full name and the phone number or email address associated with your account so we can verify your identity before processing the request.

What Gets Deleted

When you request deletion, we permanently delete your app account, login credentials, app usage data, and device information associated with your account.

What We Are Required to Keep

As a healthcare provider, we are legally required to retain certain medical and treatment records even after an account deletion request, under HIPAA, 42 CFR Part 2, and North Carolina and Virginia state law governing medical record retention. This includes:

Medical and treatment records
Appointment and visit history
Records related to substance use disorder treatment, which carry additional federal retention and confidentiality protections under 42 CFR Part 2

This information is retained for the period required by law, kept secure, and is not used for any purpose beyond what is necessary to meet our legal and clinical obligations. It is not restored to an active account or used to re-create the app account you deleted.

How Long It Takes

We process deletion requests within 30 days of receiving them.
